                                                                                                                                Ging's Medical/Physical Appointment at St. Luke's Extension Clinic

                                                                                                                                The US embassy Manila requires a complete medical check up at St. Luke's Extension Clinic for K applicants.  It's a 2-day ordeal, so be sure you take the right amount of food before going for your check up.

                                                                                                                                Here is Ging's Medical Appointment Experience:

                                                                                                                                January 10-11, 2006

                                                                                                                                My "embarrasing moments."

                                                                                                                                At 10:30am, I picked up my K packet at SLMEC. Then, I got one of the most embarassing moments I ever had.
                                                                                                                                While I was walking infront of the clinic, I took a wrong advance step on my left foot, and I had a bad slide. I fell down almost lying on the floor!! "OUCH!!!" I wasn't able to get up as soon as I can. There were 3 guys trying to help me stood up, including my brother who was with me that time, and a woman who also tried to help me. Everybody's eyes were on me....eeeekkkksss!!! I lost my poise! And that hurts really!

                                                                                                                                But anyway, back to my medical ordeal.

                                                                                                                                1:30pm - I showed my original copy of my appointment letter for interview to the entrance guard, and also showed him the photocopy of the same, after which he gave me a PDS (Patient Data Sheet) form to fill up, and he also gave me a number. Number calling wasn't followed, so I just gave my filled up PDS form to the receptionist, she verified the form, and asked me to submit the photocopy of my appointment letter, and a 1pc. 2x2 photo with my name at the back of it, and my passport.
                                                                                                                                I was told to go to a room for the computer photo capture after which I waited for my name to be called.

                                                                                                                                The lady then instructed me to proceed to to the 5th floor for payment (cashier). At that time, the rate I payed was P5,035.

                                                                                                                                After that, the cashier told me to go to 3rd floor for the Xray. They asked me the first day of my last menstrual period and asked me if I had previous or old Xray results...I said none. I was told to go to the dressing room and wore a patient's gown. (upper tops including bra are to be removed.) You don't need to remove lower clothes for this. Then proceeded to Xray room for the procedure...done in a minute..that fast! :).
                                                                                                                                I was told to go 4th floor and I went to 2nd floor instead! I laughed at my own self getting lost for a while, LOL!

                                                                                                                                At the 4th floor, the laboratory receptionist gave me first a sheet to sign (the HIV test authorization form), then waited my name to be called and she gave me a specimen cup for urine and a tube for blood extraction. I was told to go to the laboratory room at the end. The blood extraction was done first, (I was a bit nervous, my heart was pounding so fast, gosh!)...lol, but the lady had a light hand hitting my vein on my left arm. Then the urine collection after that...of course you have to do your urine catching yourself...LOL!
                                                                                                                                After the procedure, I gave back my urine specimen back to the laboratory receptionist, and she told me to go to the 5th floor for the physical examination.

                                                                                                                                At the 5th floor, I proceeded to the US counter, gave my passport and PDS to the receptionist. Eye check time...it was normal.

                                                                                                                                A nurse took my height and weight, BP, pulse and heart rate. At first, she found out, my BP was a bit high...I said I was a bit tired walking up the stairs, and I also told her I was a bit nervous. She told me to go back after I was a bit more at eased and relaxed which I complied and she took my BP again. This time it was normal :).
                                                                                                                                I went back to the US counter, and the guy there gave me room #5, and the doctor was a nice lady. Questions asked were basically the same from what asawa members had posted. The entire procedure including the undressing were done in 5 minutes.

                                                                                                                                Then next, I went to the 2nd floor for the immunization interview. The lady doctor there (with a husky voice) asked me if I want to have my immunization done here or in the US, I opted to take them here, since I already paid for it. She asked me my first day of my last MP again, and my last sexual contact...eerrr!
                                                                                                                                Then, she told me to go to the check out counter and gave my passport and PDS. The guy told me to come back January 11, 2006 at 1:30pm for my immunization.

                                                                                                                                SECOND DAY - JANUARY 11, 2006 (IMMUNIZATION DAY)

                                                                                                                                I arrived at 1:15pm. The entrance guard told me to proceed to 2nd floor. I dropped my receipt on the box provided, and waited for my name to be called. After 45 minutes, my name was called and told to go to the immunization room. I was given 2 shots, MMR and Varicella. The 2nd shot was a real "ouch". T'was hurting for an hour, and made me uncomfortable, but I was okay.

                                                                                                                                After I was given the shots, I was told to proceed to ground floor releasing counters, and waited for my name to be called. The lady at the counter asked me my passport, and 2 pcs. 2x2 photos (with my name written on the back of each photo). She asked me to sign 6 papers...(yes, I counted it, LOL!) Then, she asked me if I have ever been pregnant before, and the first day of my last MP (again for the ?th time)...she gave me my Xray result and told me to take the result with me to the US. She also told me that they will be the one to forward my medical results including my passport to the US embassy. She also gave me my copy of the Vaccination Documentation Worksheet (2 pages).

                                                                                                                                That ends my medical ordeal. Whew!

                                                                                                                                NOTE: For K applicants, you will be needing this form for your Adjustment of Status in the US later on, so don't lose it. Before adjusting status, you need to let your civil surgeon transfer this data on the Vaccination Supplemental Form (I-693) and include it on your Adjustment of Status (AOS) package.

                                                                                                                                The St. Luke's Stamp/Seal:

                                                                                                                                Below is Ging's passport where she got a stamp from SLEC indicating she completed her medical check up.

                                                                                                                                On the right page of her passport (page 31) is the St. Luke's stamp which indicates that she completed her medical appointment, including all the required laboratories and blood works, and signed by the releasing department. If you notice the date written sideways, there is the marked "N" at the end of it. That indicates medical results are normal and you have a clean bill of health. SO, if you see the mark, you need not to worry about your health.

                                                                                                                                Obviously, on the left page (page 30) is a stamp from the US embassy, acknowledging her application and all paperworks required before the interview has been received by them.
