Supplemental Form to I-693
Take note that the supplemental form is a record of your immunization/s that are to be transferred and to be signed by your civil surgeon from the form DS-3025 given by St. Luke's. It so happened that the civil surgeon (Indian doctor) that Ging went to gave her another medical examination and she said she won't transer and sign her immunization record unless Ging has her medical with her. She also requested Ging for another Xray and blood works. It sucks!..but we got no choice.
Ging's medical results, Xray and lab results were placed and sealed in the envelope by the civil surgeon. We were able to get it after a week wait.
